Updated Self-Monitoring & Wellness Screening

The Mason County Health Department has revised the self-monitoring and wellness screening since the Delta variant can cause a variety of new symptoms.

SELF-MONITORING & WELLNESS SCREENING

Currently working on-site: The District requires self-screening before each shift. If you feel ill, stay home regardless of your symptoms. Use the following self-screen questions:

“YES or NO, since your last day of work, or since your last visit to this facility, have you had any of the following:”

  1. Nausea or vomiting that you cannot attribute to another health condition?

  2. Diarrhea (at least twice in 24 hours) that you cannot attribute to another health condition?

  3. Loss of taste and/or smell?

  4. Congestion or runny nose that you cannot attribute to another health condition?

  5. Headache that you cannot attribute to another health condition?

  6. New shortness of breath or difficulty breathing that you cannot attribute to another health condition?

  7. New muscle aches (myalgias) that you cannot attribute to another health condition, or that may have been caused by a specific activity (such as physical exercise)?

  8. Fatigue that you cannot attribute to another health condition, or that may have been caused by lack of sleep etc.?

  9. A new fever (100.4°F or higher), or a sense of having a fever?

  10. Chills?

  11. A new cough that you cannot attribute to another health condition?

  12. A new sore throat that you cannot attribute to another health condition?

  13. Have you been near anyone diagnosed with COVID-19?

If you answer YES to any of the screening questions, you should not enter a PUD 3 facility and should notify your supervisor and/or HR immediately.
